The short answer
Implant healing takes months, even when the gum feels comfortable earlier. Guy’s and St Thomas’ usually waits at least three months before its implants support replacement teeth. That is a pathway example: grafting, the loading plan and clinical findings can change your own sequence.
The key points
- A healed-looking gum does not prove completed integration.
- Additional procedures can change the planned sequence.
- New or worsening symptoms deserve assessment at any stage.
Understand the different processes
The surgical wound settles while the supporting tissues adapt around the implant. The clinician uses examination and the treatment plan to decide when the next stage is appropriate. A temporary restoration may be present during this period and can have its own restrictions on loading and food.
A selected immediate-loading plan can place a provisional restoration while integration continues. That is why a photograph of teeth fitted on the surgical day does not demonstrate that the bone has healed or that unrestricted chewing is appropriate.
What affects the plan
The site, initial clinical findings, bone procedures, health factors and type of restoration can alter the sequence. Ask which milestone determines your next appointment. Do not assume that another person’s experience, an upper-versus-lower-jaw rule or a marketed loading protocol predicts your own clearance date.
- Know the purpose of each review visit.
- Follow the guidance for your temporary restoration.
- Tell the team about changes rather than waiting for a calendar deadline.
Distinguish recovery from a problem
Some postoperative discomfort and swelling can occur, but persistent, increasing or returning pain should not be dismissed as ordinary integration. Contact the practice for discharge, movement or new altered sensation. Difficulty breathing or swallowing and severe rapidly spreading swelling require emergency assessment.

Common questions
Can I tell at home whether the implant has integrated?
No. Comfort and appearance alone cannot establish integration. Do not test the implant by pushing or twisting it; let the clinical team assess readiness.
Does immediate loading mean there is no healing period?
No. A selected immediate-loading plan allows a restoration under defined conditions while healing continues. It does not grant unrestricted chewing or remove the need for review.
